Reviewer Response Tracker — instructions StatMedAnalysis | Revised 6 November 2026 Open the matching CSV in a spreadsheet or text editor. Keep an untouched copy. The first data row is a clearly labeled hypothetical example, not study data. Delete it before entering working records. Add rows without changing headers. Use public, hypothetical or aggregate information only; no patient identifiers. Use one row per reviewer comment. Record the actual check and completed change with a precise manuscript location. Keep planned work distinct from completed work. Field guide - reviewer comment: Retain the comment identifier and exact issue; mark fictional examples. - required check: Describe the analysis or source check needed to answer the comment. - action actually completed: Record only work already performed; use NONE YET for planned work. - manuscript location: Give the revised page, section, table or supplement location after checking it. - remaining limitation: State what the completed work still cannot resolve. - status: Use Open, In progress or Resolved consistently; give unresolved blockers in project notes. Decision rule Mark resolved only after verifying the change and the response against the manuscript. Report remaining limitations rather than claiming checks that were not performed. Write UNKNOWN, NOT REPORTED or NOT APPLICABLE with a reason in text fields. Leave unresolved numeric fields empty and explain them in separate project notes; never encode unknown values as zero. Store the source record and reviewer/date in your project record. This planning aid is not a validated instrument. Official guidance https://www.icmje.org/recommendations/
